Dentforms by Medictalk Software

Dentforms is a 3rd-party program that handles custom patient forms and form management. It runs as a separate program from your mainstream practice management software.

With Dentforms, you can create your own completely custom forms (although you are given basic versions of all the most common types of forms). You can then make these forms or questionaires available to your patient through a check-in kiosk computer, chairside, or through your website.

All data created is digital from the beginning, and is not converted from paper by scanning. As a result, the amount of storage space consumed for a large number of patient forms is incredibly small. A software bridge to all major software programs is included in the purchase price, and makes the integration virtually seemless.
